What should I bring to my first visit?
For your first visit, please plan to arrive at least 15 minutes early to provide information about your medical history. If you are coming from another office and have xrays less than three years old, please bring them with you to avoid taking additional films and/or delaying treatment. We will also be happy to call and have them sent to our office.

Please bring any medications you are currently taking so we can record them in your records. We will also need information about your insurance coverage. Please bring your current insurance information with you. Please bring only one person to accompany you on your office visit. This courtesy will allow room in our reception area for all our patients.


Do you accept my insurance and will my insurance cover these treaments?
Your insurance is a contract between the insured and the insurance company. As a courtesy to our patients, we will file your primary insurance for you. If your insurance has not paid the claim within 60 days, you will be billed for the balance and payment in full will be expected at that time.

We will however, continue to work with the insurance company to expedite your reimbursement. Although your insurance may assist you with partial payment of your treatment, the estimated portion, which is not covered is due when services are rendered. It is your responsibility to inform our office of any changes regarding your insurance coverage.

There is no special code for our therapy, so if your insurance company will reimburse for conventional surgery, they will reimburse for LASER PERIODONTAL THERAPY™.


When is payment due?
Payment for treatment is due in full at the completion of services. All other arrangements must be made prior to your appointment. We provide a number of payment options which may be used according to your wishes.


What types of payment do you accept?
Personal check, cashier’s check or cash. (There is a $35 fee for any checks returned for insufficient funds.) Credit Cards/Debit: Visa, MasterCard, Discover, American Express, or Debit Card. CareCredit can help you pay for treatments and procedures your insurance doesn't cover. They offer no interest* financing or low minimum payment options. What if I can't make my appointment?
If you cannot keep your appointment, please give us one day's notice. This makes it possible for another patient to see the doctor. We Reserve the right to charge for missed appointments.
